FORM V.O.1
Under Section 79 of the Local Government Act, a VO1 Form can be served upon a ratepayer by the Valuation Officer to be filled in connection with rating purposes and forwarded within 21 days. This form has to be filled in with the following information:-
- Situation of property;
- Name of occupier;
- Name of owner;
- Description of property;
- Terms of tenancy;
- Title of ownership.

FORM V.O.2
A ratepayer aggrieved of any item of entry in the valuation list or of any omission of an entry can make a proposal under Section 81 of the Local Government Act in Form VO2 and the Valuation Officer will have 6 months as from the date of the proposals to act upon.

FORM V.0.5
Under Section 82 of the Local Government Act, whenever a proposal is made by the Valuation Officer to a ratepayer to alter the Valuation List, the latter may within 28 days of the date of proposal served by the Valuation Officer, if he is not satisfied send a letter with the representations. Also in this case, the ratepayer may fill a VO5 Form, where he states the reasons for his objection.
